Vendor onboarding packet
We can send a signed W-9 and a certificate of insurance for your supplier registration. Use Request W-9 & insurance certificate in the document generator above, enter your work email, and we'll reply with the documents attached.
A note on payment security: we will never email you new bank details or ask you to change where you send payment. If you receive a message like that claiming to be from us, call (312) 361-3679 before acting on it.
Formal quotes and volume pricing
Use the document generator above, the Get a formal quote button on any product page, or the quote buttons in your cart. Quotes include our Las Vegas address, a quote number and a validity date (30 days, or 6 months for grant budgets). Volume pricing starts at 3 units. Most quotes include a Check out with this quote link, so a P-card holder can pay at the quoted price in one step.
Net-30 terms for institutions
Universities, colleges, K-12 schools, government agencies and hospitals can buy on net-30 terms: send us your purchase order and we ship right away, then invoice you with payment due 30 days from the invoice date (card, P-card, ACH or check).
- Email your PO to support@fristadenllc.com from your institution's email address, with the billing contact and ship-to address.
- First-time institutional orders over $5,000 may need a quick credit check before shipping.
- Companies and other organizations: ask us; we offer net-30 after a first prepaid order.
IQ/OQ qualification templates (free)
Regulated labs must document installation and operational qualification for new instruments. We've written fillable IQ/OQ protocols for our instruments, with our published specifications and test procedures already filled in, so your team only has to run the tests and sign. They're designed to support your own quality system; your QA team sets the final acceptance criteria.
